Job Description

Job Overview: CHWs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve. Under the direction of the Health Promotion Program Coordinator, the CHW will bridge cultural and linguistic barriers, expand access to coverage and care, and improve health outcomes. Estimated Start date: January 23rd 2024.

Major Responsibilities:

Responsibilities include, but are not limited to, the following:

  • Establishing trusting relationships with patients and their families to help them become more engaged in their health.
  • Conducting intake interviews with patients utilizing motivational interviewing and health coaching, and enrolling and/or referring patients into applicable health and community resources
  • Following-up with patients via phone calls, home visits, and visits to other settings where patients can be found.
  • Working closely with clinical teams to ensure that patients have comprehensive and coordinated care
  • Documenting patient activities and outcomes in appropriate databases
  • Support with care coordination and medical appointment scheduling for IRC resettlement clients
  • Attending team meetings as assigned to discuss progress, successes, and challenges related to the position.
  • Ability to help limited English speakers understand and become self-sufficient in navigating the U.S healthcare system including medical bills, Medicaid, primary care, and specialty care.
  • Ability to help limited English speakers understand and become self-sufficient in navigating Utah Transportation System.
  • Other duties as assigned by the clinic or supervisors must be in accordance with AmeriCorps terms and conditions.

Job Requirements:

  • At least 18 years of age
  • High school diploma or GED
  • Pass the necessary program background checks.
  • Legal Permanent Resident or citizen of the U.S.
  • Written and oral fluency in English; additional preferred language fluency in any of the following: Arabic, Dari/Pashto, Kinyarwanda, and/or Kiswahili
  • Flexibility to serve on-site and remotely as needed,
  • Basic computer skills
  • Ability to commit to approximately 36 hours per week for 11 months
  • Ability to attend a mandatory 35-day orientation.
  • Ability to develop and maintain positive relationships with patients and program stakeholders.
  • Understanding of, and sensitivity to, the target community
  • Good communication skills such as listening, speaking, and writing.
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients.
  • Ability to work independently and as part of a group.
  • Willingness to take initiative and be pro-active in daily work.
  • Willingness to travel to community-based organizations and patients homes as needed.
  • Perform Bus Orientation for clients.

Language Skills: Spoken and written fluency in English required. Proficiency in Swahili, Kinyarwanda, Arabic, or Dari/Pashto strongly preferred. 

Work Experience:

  • Relevant professional experience in health care, social services or public health field required; minimum of 1 year experience strongly preferred.
  • Relevant professional experience working with the refugee/immigrant population or underserved communities preferred.
  • Lived experience as a refugee or coming from an immigrant background may substitute work/educational requirements.

Demonstrated Skills & Competencies:

  • Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships.
  • Demonstrated success working and communicating effectively in a multi-cultural environment.
  • Self-starter with excellent problem-solving skills combined with the proven ability to prioritize and manage time effectively.
  • Attention to detail and accuracy in work product. Well Organized and able to manage own project and workload.
  • Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.
  • Valid drivers license and access to a personal, insured vehicle.

Working Environment:

  • A combination of standard office environment, remote work, and field time within the service delivery area to perform the above outlined responsibilities.
  • May require occasional weekend and/or evening work.
