Job Description:

The Clinical Manager, Case Management/Discharge Planning will oversee transitions of care for patients meeting complex discharge criteria in the inpatient and emergency departments across the full system. The Manager of Complex Transitions Care works to establish and maintain relationships with regional partners to ensure access to community services for patients and to achieve cost efficiency, quality of care, improved patient experience, and focus on population health.

Requirements:

· BSW or Bachelor’s degree in a related field required

· 3 years of experience in acute and/or post-acute discharge planning required

· Knowledge in leadership, advocacy, communication, and community resources

· Demonstrates knowledge and skills in preparing written reports

· Proficient in Microsoft Office, Excel, and other computer applications

· ACM or CCM certification preferred
