Job Summary:

Episcopal Church Home our 131 bed Skilled Nursing Facility has a rare opportunity for a Licensed Social Worker to join our Social Service team. This position will work as a member of the social service team working with a case load of long term care residents.

Job Duties:

· Coordinates social services for the resident including staff and families.

· Creates resident plan of care and treatment goals related to psychosocial needs.

· Schedules and attends care conferences for residents on caseload.

· Completes assessments and MDS charting as required.

· Serves as a liaison between the resident and the facility. Works with residents on concerns.

· Coordinates resident utilization of community resources.

· Works as a member of the interdisciplinary team.

Position Type/Expected Hours of Work: Full-Time, 40hrs/week working Monday – Friday

Minimum Qualifications:

· High school diploma required

· Licensed Social Worker

· Experience working in long term care
