Full job description
Service delivery will be delivered in a variety of settings including hospital clinics and wards, cancer charity venue and other outreach venues. The post holder will also be required to undertake administrative duties, including maintaining databases, collection of data and outcoming clinics. Part of the role will be coordinating health and well being events and supporting patients and families. Cancer experience essential.
The purpose of this post is to provide assistance to the Lead Cancer Nurse and living with and beyond cancer (LWBC) nurses to support the success and smooth implementation of all elements of the recovery package including holistic needs assessment (HNA), treatment summaries, cancer care review and a health and wellbeing event into each cancer patient pathway.
The post holder will work with site specific cancer Clinical Nurse Specialists (CNS’s) to support implementation of holistic needs assessment and implementation of treatment summaries within each site.
The post holder will be responsible for the running of the monthly LWBC health and wellbeing event for cancer patients and their carers.
The post holder will be the main link to the service to ensure that patients and professionals have a direct person to contact in relation to the health and wellbeing event thus improving communication and information sharing.
The post holder will work with site specific teams to identify suitable patients to attend the event thus increasing access to the event for all patients. At the event the post holder will provide HNA forms, support patients and carers to complete them and ensure that the forms are distributed to the site specific clinical nurse specialists for action following the screening process via HNA.
The post holder will also assist with audit and data collection in all elements of the person centred care agenda.
