Senior Services Associates, Inc. is a non-profit agency dedicated to sustaining and improving the quality of life for mature adults, persons with disabilities and their caregivers by providing access to the social services they need. We are dedicated to preserving their independence, promoting mental and physical well-being, and protecting their rights and dignity.

We are seeking a Full-Time Nursing Home Prescreener for our Aurora Office. The Prescreener will be responsible for completing nursing home prescreens at local area hospitals. Position includes benefits.

Reports To: Case Coordination Unit Supervisor

Education Requirements:

  • Must have BA/BS degrees in human services, social science, social work, or related field.

Job Duties and Responsibilities:

· Administer the appropriate intake forms, including a Choices for Care assessment.

· Maintain complete and detailed case notes. This includes documentation of all client contacts, and follow-up.

· Maintain all required department records and complete forms accurately and in a timely manner.

· Keep a daily log, maintaining records of appropriate billings and tracking time spent on each program.

· Develop and maintain a comprehensive knowledge of all relevant service and support programs needed to serve clients.

Other Job Requirements:

  • Must have personal transportation.
  • Must be willing to attend training sessions and meetings where an occasional overnight stay may be required.
  • Willing to enter into potentially uncomfortable physical settings to assist clients.

  • Recognize the nuances of a situation and have the ability to adapt his/her personal approach to the needs of the situation.
