Pay Range: $80,696 – $94,937
Summary of Position:
Circle the City is a rapidly growing non-profit community health agency dedicated to providing high quality, holistic healthcare to people experiencing homelessness in Arizona. Our homeless continuum of care is one of a kind in the State of Arizona, and one of few similar models across the nation. Circle the City patients benefit from several different levels of care offered within our organization: The sickest of our patients are offered the opportunity to rest and heal in our two state-of-the-art 50-bed medical respite centers. Additionally, patients suffering from mental illness may receive psychiatric consultation at any of our service sites, or many benefit from the more intensive, community-based services offered by our assertive community treatment team.

We engage each of our patients with the goal of not only providing excellent care, but also assisting them on their journey out of homelessness.
Essential Duties include, but are not limited to:
- Clinical Responsibilities
- Evaluate and treat each physician referred patient.
- Design and document a treatment plan based on the results of the evaluation.
- Document each visit in the Electronic Health Record (EHR).
- Document progress made toward treatment goals.
- Complete a discharge summary when goals are attained or discharged according to the wishes of the patient.
- Attend weekly multi-disciplinary rounds; participate in the discussion surrounding patients plan of care, goals and continued therapy.
- Duties and responsibilities may be added, deleted or changed to meet program needs, at the discretion of the Medical Director
- Ability to work as a collaborative member of the healthcare team
Work Location: Downtown Respite Facility at the Human Services Campus (210 S. 12th Avenue, Phoenix, AZ 85007)
Qualifications:
- Licensed to practice as a Physical Therapist in the state of Arizona required
- Master of Physical Therapy required
- Experience in working with underserved populations highly desirable
- ***COVID vaccination is required or approved religious/medical exemption.
Basic Knowledge/Skills:
- Understanding of substance and Mental Health issues
- Interpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members required
- Computer proficiency inclusive of ability to access, enter, and interpret computerized data/information required
- Basic Life Support certification required (BLS)
- Demonstrated interest in working with an underserved population
Pay: $80,696.00 – $94,937.00 per year
