Better Health Network Allied Health team has an exciting opportunity for an enthusiastic and motivated Occupational Therapist to join our multidisciplinary community based service.
We are looking for multiple vibrant and passionate Occupational Therapist to support and enhance our team on a permanent basis. Flexible hours to suit you, full time Monday to Friday (8:30am- 5pm) or part time considered, with no late nights or weekend work required. A balanced mix of from the office, providing home visits and outreach services, and opportunity to work from home.
We see adults across the My Aged Care, Home and Community Care and Community Health funding streams. BHN provides support and mentoring and can support upskilling where required to bridge gaps in your knowledge.

Responsibilities:
- Provide comprehensive occupational therapy assessments to clients, including those with multiple and complex needs.
- OT services include home assessments, advice on activities of daily living, home modifications, and provision of assistive equipment.
- Care plans should be developed in collaboration with clients and carers.
- Provide support, education and direction to Allied Health Assistants, including overseeing the ordering of OT equipment and supplies.
- Submit individual applications for home modifications and equipment through the Statewide Equipment Program (SWEP) or other funding agencies as appropriate.
- Maintain current knowledge of target population, funding sources and referral systems including My Aged Care / Commonwealth Home Support Program (CHSP), Home Care Packages, Home and Community Care – Program for Young People (HACC PYP), Community Health and National Disability Insurance Scheme (NDIS).
About you
- Bachelor of Occupational Therapy or recognised equivalent qualification and Current registration with the Australian Health Practitioner Regulation Agency.
- Provided OT services to clients with diverse backgrounds and multiple and complex needs.
- An ability to provide specific occupational therapy services and programs, including home assessments, advice on activities of daily living, home modifications, and assistive equipment.
- State Wide Equipment Service (SWEP) registered or eligible for registration as an equipment prescriber.
- Knowledge and experience of My Aged Care / CHSP, HACC PYP, Home Care Packages and NDIS.
- Providing self-management support and education to clients. Commitment to the philosophy of Community Health.
- Understanding of chronic disease management approaches and social models of health.

About the team
Based at our Prahran community health centre, our OT and wider allied health teams are highly skilled and supportive, and we promote opportunities for engagement and collaboration. Clinicians looking to move into a new area will be assisted and well supported in the transition and those with established experience will be encouraged to share their knowledge.
The team has a strong and shared sense of the social model of health and working with vulnerable cohorts.
