The need-to-know
As a Community Support Worker (Disability) at Rise, you will support clients, their families & carers to develop skills & capacity to promote independence within their community. Your day will involve driving to multiple client’s locations & helping them to identify & achieve their goals using a combination of person centred, strengths based, role modelling and mentoring approaches. You will work with your clients to explore, engage, and build upon existing interests to enhance their social and economic participation and report on progress, achievements, and concerns.
We have various shifts available across Monday to Saturday, working approximately 60 hours a fortnight. Examples of available shifts are 8.00am to 1.00pm, 10.00am to 3.00pm and 3.00pm to 9.00pm in various Perth suburbs including Armadale, Midland, Canning vale, Como & Bassendean.
You will be reimbursed for travel time & kilometres between clients and during a service.

Please note that there are no Sunday or night shifts available.
The need-to-have…

- Hands-on experience in disability support work (ideally within the community or a keen interest to transfer from supported living)
- Experience with providing positive behaviour support.
- Driving licence and own vehicle to use for work purposes.
- Willingness to travel across Perth Metro areas to clients.

Appointments to this position will be subject to a NDIS Worker Screening Check / a driving licence and reliable vehicle to use for work purposes.
Costs for new checks will be covered and/or reimbursed by Rise.
