About the role

The position is based at the Integrated Pharmacy Services Collingwood Pharmacy (cohealth pharmacy) within cohealth Community Health Centre in Collingwood. The pharmacy operates a clinic dispensary model, and the pharmacist works as a member of a multidisciplinary team in the community health centre. The Centre supports clients from the local municipalities and priority populations. Patients, carers, medical and allied health professionals and community support agencies are provided advice and education by pharmacists to integrate medication therapy with other health and social intervention and support.

Your duties include:

  • Comply with relevant State and National legislation and regulation relevant to the practice of pharmacists and with Pharmacy Board of Australia standards and guidelines
  • Dispense prescriptions from doctors and other licenced prescribers
  • Counsel patients on the correct and effective use of medications
  • Establish and maintain records of all medication dispensing in accordance with statutory requirements

Our ideal candidate:

  • Current registration as a pharmacist with AHPRA
  • Ability to work as a collaborative member of a multi-disciplinary team
  • Excellent communication and interpersonal skills
  • Excellent organisational and time management skills

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
