About the Role

  • This is a permanent full-time opportunity working 38hours / week across a 7-day rotating roster.
  • Shift times are between 8:00am – 4:00pm OR 12:00pm – 8:00pm
  • Located in Chatswood with travel across Hornsby, Ryde, Northern Beaches and the lower North Shore

Join our dynamic team in Chatswood as a Mental Health Support Worker, where you’ll provide vital community-based outreach and recovery-oriented support to individuals aged 16 and over. This role is pivotal in empowering individuals with varying mental health concerns to achieve greater independence and improve their overall quality of life. 

About you in the role

  • Provide one-on-one and group support as per the shared schedule with colleagues, ensuring a balanced and collaborative approach to care.
  • Develop and implement individual care plans tailored to the needs and goals of each consumer.
  • Plan and facilitate group activities that promote social engagement and skill development.
  • Maintain accurate documentation and records using CareLink, ensuring that risk assessments, care plans, and progress notes are up to date.
  • Collaborate closely with external clinical teams, including the Local Health District (LHD), to provide monthly summaries, attend meetings, and adjust care plans as necessary.
  • Liaise with referral sources such as LHD, hospitals, Community Housing Providers and correctional services to ensure a seamless transition for consumers entering the program.
  • Support consumers in achieving daily living skills, maintaining a positive lifestyle, accompaniment to appointment and engaging with employment or education services. 

About You

  • Minimum Certificate IV in Mental Health or Community Services
  • A current full driver’s license and comfortable driving across Northern Sydney.
  • Passionate About Mental Health: You have a genuine commitment to supporting individuals on their recovery journey.
  • Strong Communication Skills: You can effectively engage with clients and collaborate with team members and external partners.
  • Understanding of the Recovery Framework: Experience with or a desire to learn about recovery-oriented approaches
  • Risk Assessment Ability: You can identify and respond to potential risks, ensuring the safety and well-being of clients and workers.
  • Administrative Proficiency: Comfortable with the balance of community work and the administrative demands of the role.
