- This is a full time (35 hours per week), closed period contract until June 2025.
- This role will be based in Campbelltown but will work across the broader South West Sydney region.
About the Role
This is an exciting opportunity to be part of a new initiative in partnership with Karitane and South West Sydney Local Health District developed to support young parents and their children.
As a Key Worker you will manage a caseload (15 families) of vulnerable young parents by identifying services through a stepped model of care to address their individual needs by providing information, advice assessment case planning, service coordination and engagement.
Key responsibilities include:
- Deliver services using evidence based frameworks, approaches and models
- Engage young parents in a flexible and responsive way including home visits, cold calling and meeting families in community environments
- Provide planning, support and structure for child and family safety
- Coach and support the capacity building and independence of young parents
- Develop and maintain update to date knowledge of local service systems (SWS) and build relationships with relevant service providers
- Engage in a partnership approach to supporting young parents and building their network of support
About You
- Diploma in Community Services with relevant experience OR lesser qualification in Community Services with at least 3 years relevant experience
- Demonstrated experience working with and challenges faced by young parents, experiencing disability, CALD, refugees, mental health conditions, family and domestic violence, ATSI and low socio economic
- Demonstrate understanding of child protection issues and work with families with child protection concerns
- Experience working with a range of practice frameworks, approaches and models of support
- Proficient in conducting needs assessments, develop case plans using a strengths based approach
