Role Description

The Clinical Case Coordinator will support the Child and Youth Assessment & Therapeutic Service (CYATS) to ensure that children/young people and their families referred for assessment/treatment receive comprehensive and responsive services, by playing a key role in the collection of background information about clients, liaising with key internal and external stakeholders and managing other administrative tasks as required. The service aims to provide children and young people with neurodevelopmental vulnerability access to seamless assessment, treatment and management and support access to the National Disability Insurance Scheme through a multidisciplinary team (MDT)

To be considered for this position, you will require:

  • Minimum of diploma qualified or relevant experience in a clinical case coordination role or similar.
  • Demonstrated experience and skills in providing clinical case management support to vulnerable families that is child focussed, family centred and trauma-informed.
  • Knowledge of child development, as well as neurodevelopmental disorders including FASD, ASD and Global Development Disorders
  • Demonstrated ability to foster and maintain collaborative working partnerships with internal and external providers.
  • Awareness of and sensitivity to Aboriginal culture and history, and knowledge of factors influencing the health and development of Aboriginal children and adolescents.
