About the position

The Access Outreach clinician role includes clinical service delivery utilising HEADSS and risk assessments, care and safety planning, psychosocial education and brief therapeutic interventions delivered both on site and in an outreach capacity. The outreach model will utilise a 6 session model with a case load of 6 clients.

Highly developed engagement and communication skills with young people who are experiencing multiple and complex social disadvantages and a demonstrated ability to motivate them to develop and achieve their goals.

Skills in engaging and working with young people (and their families), ranging from frontline advocacy, conducting assertive outreach, case management, and family focused interventions to achieve practical and meaningful outcomes.

Ability to conduct assessments (mental state examinations, health and well-being, risk assessments), formulate interventions, develop and implement case plans based in evidence and aligned to Resilience based practice.

Demonstrated ability and experience in working with families.

This position will also work as part of the Access Team, who provides comprehensive intake, triaging, assessment; mental health support and psychological therapy to young people aged 12-25. This multidisciplinary team serves as the first point of contact to the service, it is critical that they demonstrate a high level of customer service and professionalism in their approach.

Some of the key selection criteria will be:

  • An understanding of the social and health needs of young people
  • Demonstrated skills and experience in triage, assessment, care planning, psychological interventions and referrals for young people, families and groups
  • Demonstrated experience in the delivery of clinical and non-clinical services for young people with mental health issues and other psychosocial needs
  • Ability to contribute to a multi-disciplinary team and to establish effective relationships with staff, clients, carers and workers in community agencies
  • Highly developed interpersonal and communication skills, including sound oral and writing skills, and practical knowledge of an familiarity with computer applications
  • Maintain professional relationships with tertiary mental health services and other relevant service providers to support seamless service delivery.
