About the position
The Access Outreach Senior Practitioner is based in the Access Team and assists in the intake and allocation of new referrals, clinical review and quality assurance processes within supervision of the Access Team. This role includes clinical service delivery utilising HEADSS and risk assessments, care and safety planning, psychosocial education and brief therapeutic interventions delivered both on site and in an outreach capacity. The outreach model will utilise a 6 session model with a case load of 6 clients.
In addition, opportunities are available to perform supervisory duties with students depending on qualifications. They will be expected to contribute to the development and maintenance of community partnerships and referral pathways. The Access Outreach Senior Practitioner contributes to clinical governance, escalation and support at the centre alongside the clinical team leader.
Some of your key selection criteria’s will be:
- Demonstrated experience in the provision of evidence based therapeutic approaches to young people and their families with mental health and/or drug and alcohol issues, with a sound knowledge of and experience in working with complex young people and their families in both an intake, case management and therapeutic role
- Demonstrated skills in problem solving and negotiation skills especially with young people, families and service providers in the early stages of help seeking.
- Strong leadership skills and experience required to work alongside senior management, staff and stakeholders
- Desire to develop and mentor Clinicians/students early in their careers, to ensure quality service provision and knowledge transfer
- Highly developed interpersonal and communication skills, including oral and writing skills, record keeping/data management and use of computer applications
- Maintain professional relationships with tertiary mental health services and other relevant service providers to support seamless service delivery.
