Adult Social Care Practitioners in the mental health team work closely with their health and community colleagues to gain an in-depth understanding of community resources that can facilitate and build independence, maintaining people’s ability to remain living in their own community networks and working with a strengths-based approach.Our goal of working with people to develop gloriously ordinary lives remains the same throughout our practice which means:
The teamThe Mental Health inpatient team is made up of 6 social workers and 4 adult social care practitioners and is overseen by an advance practitioner. We cover all 9 NHS Psychiatric hospitals across the whole of Somerset; supporting individuals prior to discharge from hospital.The service offers a hybrid way of working and due to covering the whole of Somerset, the team work hard to ensure time together is scheduled on a regular basis. This includes, shared lunches, cuppa and catch-up time, as well as meals/drinks in a social aspect.All members of the team are supported to progress within the team, should they wish. This includes training in specialist areas, such as BIA, Practice educator or progression to become an advance practitioner or Approved Mental Health Professional.Some feedback from the team around the best part of their job are that they get to work with people and watch their improvement, both physically and mentally. As well as being able to support people to realise their own potential and set goals for themselves.Working within the Mental Health inpatient team is a fast-paced environment, but you are guaranteed fun and laughter along the way due to the positive and fun nature of the team.To be successful in the role it will help if you:

Full and part time hours can be considered depending on team and location.You will need to be able and willing to travel countywide, including to areas that are not currently serviced by public transport.This is a Grade 12 post with a current salary of £26,421 to £29,777 per annum.
