The Aged Care Support Coordinator role will support clients to navigate access to appropriate local aged care support services to ensure the client’s care and safety needs are met. This full time fixed-term position is till 30 June 2025, and is based in Darwin or Nhulunbuy NT, with regular travel required to MHAC health centres located across East Arnhem Land.

- A competitive base salary of $79,695 – $91,486 per annum pro rata + 11% Superannuation.

Miwatj Health is an independent Aboriginal Community-Controlled Health Service. Established in 1992, we’re committed to securing quality health care services across the East Arnhem and enabling powered community health and wellbeing. Miwatj provides culturally appropriate, comprehensive primary health care to over 6,000 remote Aboriginal and Torres Strait Islander residents across 3 East Arnhem wards. With a Head Office in Nhulunbuy, Miwatj also manages a Corporate Office in Darwin, 7 fixed clinics across the East Arnhem region, and dedicated outreach teams who travel to 3 additional locations.
Essential Requirements
The Aged Care Support Coordinator will be responsible for advocating for client’s needs, supporting clients to identify available aged care services and access pathways, and supporting clients through the aged care assessment process. Candidates must have demonstrated understanding of the principles of person-centred care and trauma-informed practice; and experience working within the aged care industry. They will also have an understanding of and commitment to Aboriginal Community Control, self-determination, and the issues faced by people living in remote settings.
