About the role

These positions sit within the multidisciplinary team at the Brimbank Mental Health and Wellbeing Local and they work closely together with a strong focus on outreach in addition to onsite services at the Brimbank site.

The Assertive Outreach Worker will receive day to day and line management support from Brimbank Local Operations Manager and will also maintain regular connection with cohealth’s Homelessness and AOD programs including receiving support from the Specialist Team Support: Homelessness Health or the Specialist Team Support AOD Response West.

Your duties include:

  • Conduct assertive outreach to locations where people experiencing an episode of homelessness and/or use substances are known to frequent – to engage and link them into the services they require
  • Work with dual diagnosis clients
  • Provide care and recovery coordination services as required, including care planning and timely referrals to support services
  • In conjunction with the Brimbank Local multi-disciplinary team, provide crisis intervention management for complex and challenging clients

Our ideal candidate:

  • A relevant tertiary qualification and/or significant relevant experience in the housing, AOD, mental health, disability, or welfare sectors
  • Demonstrated experience in working with clients who have a history of homelessness and multiple and complex needs
  • Ability to complete comprehensive AOD assessments with clients and undertake care & recovery coordination as required
  • Sound understanding of challenges people with mental illness and their families face with a knowledge and understanding of recovery models of care, psychosocial support, and best practice evidence-based interventions

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses).

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
