You will ideally be located in the East/ North East of Adelaide and love to travel. Travel throughout the region may be required.

What is the role?

As an Assessor with ACNA you will join us to deliver our purpose and share our values. You will bring your commitment to wellness and holistic, strengths-based practice through daily assessments of our My Aged Care clients in their home.

What will a typical week look like?

  • Travelling to our client’s home to conduct the My Aged Care assessment as part of the Regional Assessment Services (RAS) program.
  • Applying ACNA’s Reablement Approach to assessments, developing strategies and supports that empower clients and support their independence.
  • Communicating in person, over the phone and in writing with clients, families, service providers, your team and other program contacts during the assessment process.
  • Developing support plans, generating assessment summaries and case notes that document the client’s journey.
  • Working as part of a skilled team to expand your knowledge, networks and career.

Who are we looking for?

People have called you all sorts of names. Authentic. Fun. Genuine. Brilliant. Approachable. A bit cheeky. Helpful. Caring. Lovely. Relentless.

In your spare time we’re likely to find you daydreaming about ways to create an impact on people’s lives.

You are a great communicator and conduct assessments focusing on the capacity of an older person.

A diploma or higher-level qualification is highly regarded.

Sound interesting?

If you have experience as an assessor in community services and love being busy, consider joining Access Care Network Australia (ACNA), a forward-thinking and innovative not-for-profit.

We want to ensure you are a great fit for ACNA, and are looking for people that align with our values of supporting people to be their best and always striving to learn, improve and grow. Hiring people whose values align with ours enables us to build awesome teams who create the best outcomes for our clients.

About ACNA

Our charter says…

Access Care Network Australia provides assessment, coordination and case management services to connect eligible people with supports that build on their strengths.

We’re in business to change lives.

We do that by helping clients (and their families) get the support they need to live life their way.

What does it take to do what we do?

3 things. People. People and people.

We hire only in the words of clients – “amazing beautiful souls” and never break that rule. We hire when we are confident people have what it takes to make a difference. Think you do? Then join us!

We love all ages and life stages. If you need a little flexibility and want to be part of a team that bubbles with life experiences, that’s us!

We measure success in people, not data points. Data points don’t capture the life-changing impact of an assessment that gives a daughter’s 92-year-old father the renewed confidence to live independently. We get a front row seat to these amazing client stories that remind us why we’re here.

What makes us tick?

We want people to be their best.

This isn’t something we write on a poster: ‘being our best’ is our way of life. It’s a life-long personal commitment to be…

Always learning

Always growing

Always sharing
