We have the opportunity for a committed and friendly Nurse to join Chippenham Community team as one of our Community Nurse Specialist. We are looking for an inspirational Nurse to deliver a high standard of quality care to our patients and support the team with excellent leadership skills.Our community teams deliver nursing care in patients homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their health conditions to prevent hospital admission and promote independent living and health management.Applicants need to have excellent clinical skills, well developed communication skills, be experienced in leadership and management and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently.
