Job Summary

We are seeking a dedicated Patient Navigator to provide non-clinical support, guidance and assistance to connect under-resourced patients and families to resources and support. This person will review patient responses to a Social Drivers of Health Questionnaire, route patients to appropriate support based on patient level of need, and guide patients to a broad range of services, amenities, and information. This role will also be responsible for tracking referrals, following up with patients to document issues and barriers to accessing services, and creating reports for management to review for planning purposes.

The job will require close collaboration with Social Work and the Office of Community Engagement. The work centers around patients seen at UCSF Berkely Outpatient Center and will be performed on site and remotely.

The final salary and offer components are subject to additional approvals based on UC policy.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% – 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

The Department of Epidemiology & Biostatistics is committed to advancing health for all through rigorous, innovative, multidisciplinary research.

Required Qualifications

  • Bachelor’s degree and/or equivalent experience/training.
  • Attention to detail; strong interpersonal skills; excellent, effective verbal and written communication skills to coordinate with patients and team members; and the ability to multi-task in a fast-paced environment while working with a diverse subject population.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to work well independently, complete projects in a timely manner, and prioritize multiple projects to ensure the completion of essential tasks by deadlines.
  • Team player who collaborates effectively with colleagues.
  • Proficiency with Microsoft Office 365, including Word, Excel, Outlook, and PowerPoint.
  • Ability to pass a background check.

Preferred Qualifications

  • Fluency in Spanish, Cantonese or Mandarin
  • Experience with EPIC Systems Inc. software (electronic health record)
  • Experience working in community outreach or non-profit settings
