Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.
About the Role
We are looking for a motivated Nurse who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander people. The successful candidate will provide a high-quality service along with contributing to the development of models of holisitic health incorporating continuity of care. As part of Kalwun’s ongoing growth and development, you will be joining a community-focused team at the Bilinga clinic.
Position Duties
- Provide care coordination for the clients within your clinic.
- Deliver a high level of clinical care, including the timely provision of clinical services to patients which may include the screening of patients in the clinic setting or in an outreach setting to assist other clinical staff or as directed by your Practice Manager.
- Arrange the medical, specialist and allied health services required for ongoing care.
- Transferring and or updating of a patient’s medical records.
- Ensure a collaborative and multidisciplinary approach with all Health staff to ensure a holistic health approach is taken to all patients.
- Assist patients to participate in regular reviews by their GP.
- Follow-up of patient recall and reminder systems, such as but not limited to: abnormal results and care plan tasks.
Essential Selection Criteria
- Relevant clinical qualifications for an Enrolled Nurse is preferred (Diploma of Nursing)
- Current AHPRA registration and renew annually
- Maintain appropriate CPD points
- Demonstrated knowledge of mainstream primary health care services and the patient journey
- Sound knowledge of health issues affecting Aboriginal and Torres Strait Islander people and a strong commitment to improving health issues
- Demonstrated experience in coordinating internal and external service providers to ensure that services are managed efficiently
- Be able to provide a culturally appropriate and culturally sensitive service to patients
- Demonstrated understanding of the general practice sector and an ability to work within mainstream primary care providers
- Sound understanding of clinical policies, systems and procedures
- Sound knowledge of the Medicare Benefits Schedule and its application in the CCHS setting, including understanding of the Closing The Gap programs and targets
- High level of communication skills that demonstrate the ability to effectively negotiate and liaise with clients and members of the community, health professionals and GP clinic staff in the provision of professional quality client service
- Current Qld C class drivers license
