Organisation Description:

The Australian Childhood Foundation stands at the forefront of transformative reforms in Western Australia’s Out-of-Home Care (OOHC) sector. Aligned with the WA Government’s initiatives, the Foundation emphasises stability in care, and cultural sensitivity, and aims for seamless transitions and strong cultural connections for young people. Awarded a contract to deliver foster care services in the Perth area, the Foundation collaborates closely with Karla Kuliny Aboriginal Corporation to provide trauma-transformative therapeutic services, making significant strides in improving care outcomes for children and young people.

About the Role:

The Care Coordinator will be responsible for facilitating and coordinating the care and support for children and young people within the Out of Home Care (OOHC) program. This position will work collaboratively with carers and families, a team of Therapeutic Specialists, other agencies/service providers, and the Department of Communities to holistically support the needs of children and young people in the program. This role will also be responsible for the implementation of developed individual plans as well as coordinating external referrals where appropriate. The successful candidate will take an integrated case management approach that will be based on a shared understanding of the risks and needs for support of young people in out of home care.

The Foundation currently has a number of full-time opportunities available for Care Coordinator’s based in East Perth on an ongoing basis.

Key Selection Criteria/About You:

For this role, you should possess:

  • An appropriate tertiary qualification in Psychology, Social Work, or a related discipline is preferred.
  • Demonstrated experience in a similar role in the out of home care sector.
  • Previous experience in a case management role.
  • Strong understanding of the policies governing the out of home care sector.
  • Ability to navigate complex situations and make informed decisions.
  • A knowledge of trauma, attachment and neurobiology frameworks and theories.
  • Substantial experience in the area of child abuse and/or in working with traumatised children.
  • Demonstrated commitment to working collaboratively and the capacity to negotiate and liaise with volunteers, including carers, Department of Communities, other agencies, and the community.
  • A sophisticated understanding of the complexity of the service system.
  • Personal values consistent with the values of the organisation.
  • A current driver’s licence and Working with Children check or equivalent.
