Uniting enables communities to thrive through improved health and wellbeing and we are on the lookout for our next Care Finder to join our team in Berrigan, NSW. This is a full-time position.
The Care Finder role:
- Provides specialist and intensive assistance to help people understand and access aged care
- Helps remove barriers for aged care recipients in connecting with relevant community supports
- Supports improved integration between health, aged care and other systems at the local level
- Connects and engages older people who have significant difficulty accessing aged care services.
- Communicates and collaborates with participants, carers/families and relevant stakeholders.
Our ideal candidate will have the following:
- Qualifications in Allied Health, Nursing, Community Services, Social Work/Social Science or equivalent
- Working knowledge of the ageing process and related life transitions
- Understanding of aged care supports and services available in the local and regional communities
- Commitment to delivering a person-centred approach that respects and responds to each person’s individual needs preferences, values and life experiences
- Well-developed communication and interpersonal skills and ability to engage and communicate effectively with people from a range of backgrounds
- Strong solution focused, problem-solving skills
- Strong administrative skills, including an ability to use and accurately enter information into client management systems, databases and/or portals.
- Current Australian Drivers Licence and a willingness to travel occasionally for work purposes.
