About the role:
As the Care Management Team Leader at Home Instead Melbourne Inner South and Bayside, you will play a pivotal role in fulfilling our mission: “To enhance the lives of ageing adults and their families.”
In this role, you will lead and support the Care Management Team, ensuring they achieve their individual goals while maintaining the highest standards of compliance and clinical governance. Your leadership will be instrumental in delivering quality client care services that elevate the overall client experience, ultimately making a positive impact on the lives of ageing adults.

Key Responsibilities:
- Represent the business as a leader in aged care and advocate for our brand within the community.
- Provide daily leadership and regular performance management to Care Managers to support an effective CAREGiver workforce and ensure client satisfaction.
- Review clients to drive business growth, implement strategies for future growth, and identify opportunities for service improvement.
- Manage care operations, maintaining accurate client records in RMS, My Aged Care, and the Aged Care Provider portal, including new clients, discharges, and monthly claims.
- Review and approve Care Plans, ensuring clinical needs and management plans are properly recorded and addressed.
Qualifications & Experience:
- Registered Nurse with a bachelor’s degree in nursing with AHPRA registration.
- Experience leading teams and supervising team members.
- An understanding of the Aged Care Service System and Home Care Packages
- Highly efficient and organised with proven time management skills.
- Must have great communication and attention to details skills.
- Self-motivated and outcome orientated, with the capacity to deal with multiple and at times conflicting priorities.
- Must possess a valid driver’s license.
