Your new Role:
The Social Justice Services (SJS) team, based in Epping, is recruiting a part time Care and Recovery Coordinator to support clients with substance issues and mental illness in their recovery journey. This position will compliment the therapeutic services provided by SJS clinical staff by providing case management and care coordination to clients.
Responsibilities will include assessment of needs, assertive referral, individual support, education sessions, linking clients with local services, liaising with other health and welfare professionals and building relationships with local services. The role will be based predominantly onsite at Caraniche’s Epping office with the ability to negotiate some remote work as it aligns to team and business needs.
The ideal candidate:
- Demonstrated experience and knowledge of working with clients with drug and alcohol issues and/or clients with offending histories
- Previous experience working in a care coordination or case management
- Ability to develop relationships with other service providers
- Highly developed administrative skills including strong computer skills
- Excellent communication skills
- Sound knowledge of the of the AOD sector within Victoria
- Previous intake or triage experience
- High level of interpersonal skills
