Scope of Job | Responsible for coordinating and facilitating services to clients receiving mental health care through the Counseling and Psychological Services center as well as services provided by other Dean of Students offices. |
Description of Duties | • Provides casework services to include the assessment of need, formation of plan, intervention, and follow-up monitoring of individual student status. • Responsible for establishing relationships with patients while providing general support and individual case management services from entry through discharge. • Interviews and screens patients to gather information to assess service needs. • Assists in the development and implementation of treatment plans to meet patients’ needs. • Coordinates and monitors services, including comprehensive tracking of patients’ activities in relation to care plan. • Maintains case history and progress records; prepares reports as needed. • Facilitates and coordinates access to campus and community social service resources. • Conducts periodic reviews and follow-ups to evaluate quality of clients’ referral experiences and subsequent care. • Establishes and maintains relationships with referring campus and community agencies. • Serves as liaison between patients and campus/community providers. • Performs other duties as assigned. |

Required Education | Bachelor’s degree in social work, counseling, psychology or related field from an accredited university. |

Other | Excellent interpersonal communication skills. Ability to manage multiple, competing priorities. Knowledge of community resources; of case management principles, objectives, standards, and methods. Ability to assess clients’ needs, to coordinate clients’ services, to communicate effectively. |
