We are seeking a passionate and experienced Case Manager to join our team! In this role, you will collaborate closely with ONCALL team members to assess, monitor, and review client needs, ensuring that best practice approaches are delivered.

Key responsibilities:

  • Work effectively with clients, their families, and other stakeholders to deliver high-quality, person-centred care.
  • Attend and participate in case conferences, employee, and stakeholder meetings, documenting outcomes and complete actions within specified timeframes.
  • Maintain accurate and timely records in compliance with policies and procedures.
  • Develop care plans and agreements in collaboration with clients and stakeholders.
  • Prepare support letters and recommendations for clients for governing body reviews.
  • Monitor and evaluate service delivery through quality improvement measures.
  • Manage client issues and respond to feedback effectively.
  • Provide holistic and individualised case management within a person-centred framework.

From you, we need:

  • Tertiary qualification in Nursing, Allied Health, Disability or a related field preferred.
  • Experience in the disability or health sector, with an understanding of the challenges faced by people with disabilities and their families.
  • Sound knowledge of the NDIS scheme, including standards, policies, and procedures.
  • Proven experience in planning, case management, and coordination of services for individuals with complex needs.
