About the role

We are seeking a dedicated Case Manager for our St Kilda Contact Centre! You will provide a high-quality response to clients with complex needs in housing crisis who present at the St Kilda Contact Centre, Access Health and Housing Entry points who require assistance with housing and support plans.

Responsibilities include

  • Provide intensive, housing focused, short term, case management support to clients
  • Develop and review individualised case plans with clients and assist them to implement strategies to achieve their goals by building on their strengths
  • Initiate contact with clients using flexible engagement strategies to build positive relationships that support self-determination and achieve their case plan goals
  • Deliver a high-quality service by maintaining constructive and supportive relationships with clients by working within Person- Centred, Strengths Based and Trauma-Informed models of care
  • Facilitate client access to community resources and services, work collaboratively with allied health services to achieve an inclusive and coordinated response to client needs

This is a permanent full-time position, based in St Kilda, VIC. Salary and conditions are in accordance with SCHADS Award Level 5.

You will have

  • Tertiary qualification in Social Work or a related discipline (considered desirable)
  • Experience working in the Homelessness sector and community services
  • Understanding of the Victorian Homelessness and Housing service system, including legislative requirements
  • A strong ability to respond to the needs of individuals experiencing housing, homelessness and/or tenancy issues
  • Knowledge of the housing sector including role, tenancy law and legislation
  • Ability to work effectively with individuals in a case management process, reviewing case plans and provide strength-based support to assist individuals, families
  • A high level of personal initiative and motivation and acts in a decisive way
  • High level of administration skills
  • Current Australian Driver’s License (required)
  • Disclosure of pre-existing health conditions that may impact on fulfilling position requirements
