Role of the Case Manager: SEED case managers deliver services to protect, empower and support the recovery of vulnerable individuals in Kurdistan through comprehensive case management and psychosocial support for survivors of violence and others at risk.  The case manager will work directly with clients to holistically address their needs consistent with international standards, protocols for Iraq and Kurdistan, and SEED’s policies and procedures. 

 

While all case managers will receive training and supervision, successful candidates must be compassionate, confident, diligent, trustworthy, resourceful, interested in learning new skills, and able to work both independently and as part of a team. The case manager must be willing and interested to work with team members and clients of different religious, ethnic, cultural, educational, and socio-economic backgrounds. All services will be provided in accordance with international ethical standards.

 

This position reports to the Service Delivery Coordinator.

 

 

Specific Tasks:

Provision of Case Management Services (70%)

  • Provide confidential ongoing case management to clients and their families, as needed.
  • Case management services will include conducting comprehensive  needs assessments, collaborative development of a “care plan” across SEED’s six domains of care: mental health, physical health, education and livelihoods, protection and legal, shelter, and support networks, and make referrals for psychosocial, mental health and legal support in accordance with the survivor centered approaches.
  • Work in collaboration with other service providers in SEED, such as psychologists, on all services.
  • Facilitate provision of direct services to clients by SEED based on their needs and vulnerabilities including but not limited to food baskets, financial assistance among others
  • Facilitate referrals to other agencies providing services in accordance with the Standard of Care and follow up regularly, as needed per each individual case until all the needs are met.
  • Where necessary, accompany clients to various service centers including but not limited to the police station, medical appointments, and other government services to ensure that clients’ needs are met in a timely and appropriate manner
  • Ensure complicated cases are identified and raised to the Service Delivery Coordinator.
  • Ensure regular client visits and all services are in line with SEED’s Standard of Care and other policies.
  • Ensure proper case documentation including sessions notes.
  • Participate in supervision sessions with the Service Delivery Coordinator and Program Manager, as required, and case conferences with the Service Delivery Coordinator, Program Manager and other team members as required.
  • Support the process of regular mapping of the existing services in the community.
  • Attend relevant training sessions, as assigned, to ensure that the highest quality services can be delivered.
  • Perform other tasks as assigned by the Service Delivery Coordinator and the Program Manager.

Provision of PSS Activities (15%)

  • Deliver psychosocial support activities including facilitating emotional support group sessions, as relevant to the population being served, structured recreational activities, psychoeducation and life skills, on as-needed basis.
  • Participate in the development and implementation of outreach and community engagement plans.
  • Participate in awareness creation in the community through different media as directed by the communications team and program manager
  • Facilitate equal participation of men, women, boys and girls in PSS activities with SEED centers

 

Documentation & Reporting (15%)

  • Ensure documentation of all interactions with clients or other contacts in relation to clients’ cases in accordance with SEED’s case management system, on an ongoing basis.
  • Ensure accurate and ongoing updating of all case management activities (completing of all required forms) on CommCare database
  • Accurately measure improvement in well-being and mental health through service provided.
  • Collect and document honest feedback from clients regarding services provided to the and the level of satisfaction
  • Follow SEED’s data collection approaches and ensure systematic gathering of data.
  • Provide accurate, clear, and precise quality data of clients’ cases and other activities that contribute to monthly service delivery report and  grant reporting.
  • Report on monthly activities in a timely manner.
  • Where appropriate, provide impact/success stories from clients served.

  • University degree in social work, psychology or a related field.
  • Minimum of 2 years of professional experience as a social worker, caseworker, or case manager within an NGO.
  • Experience providing case management, basic psychosocial support services, experience providing psychological first aid. Working in a protective shelter environment is a plus.
  • Experience working with survivors of GBV, trafficking, and other populations experiencing a lack of social protection and social inequalities desirable.
  • Strong belief in human rights and gender equality.
  • While all case managers will receive training and supervision, the case manager must be compassionate, confident, diligent, trustworthy, resourceful, interested in learning new skills, and able to work both independently and as part of a team.
  • Good interpersonal skills including the ability to gain trust and build relationships with team members and clients.
  • Strong computer skills.
  • High standard of spoken and written English, Arabic, and Kurdish.
  • Ability to work in a stressful setting and adapt quickly to changing environments.
