Job Description:
POSITION GOALS
The Case Manager/Care Coordinator will demonstrate and promote a culture of diversity, equity and inclusion on all campuses. They will focus on promoting holistic student wellness through providing early support and follow-up for complex situations involving student concerns. The Case Manager/Care Coordinator will monitor students and develop action plans to facilitate referrals to on-and off-campus resources. In addition, they will develop and lead programs, trainings, and workshops for faculty, staff, and students that support holistic student development and success.
ESSENTIAL FUNCTIONS
- Provide case management and care coordination of support services, including referral and liaison with housing agencies, social services, mental health services, financial support agencies, food services, and other basic needs resources for students.
- Develop and maintain a database of referral resources for social service agencies within the local community; facilitate relationships with off-campus agencies and resources to connect students to long-term support.
- Conduct initial intake meetings with students to triage and assess needs for appropriate referrals within and outside the College and establish expectations for future meetings.
- Serve as a BIT (Behavioral Intervention Team) Team member and case manager to ensure the timely management and resolution of referred students needing mental health services, including referrals to crisis counseling, short-term counseling, psychiatric support and evaluations, and other community support services.
- Assist academic deans, department chairs and faculty regarding individual students and behaviors that affect classroom management.
- Maintain confidential student records.
- Manage and support students’ return to academic and campus environments following significant events and the re-entry phase for students who return from a leave of absence (LOA), suspension, or other significant events.
- Conduct crisis intervention for students with immediate definable needs such as shelter, food, financial, transportation, and legal support.
- Develop and coordinate educational information and related activities campus-wide to increase health and wellness awareness.
- Plan, develop, and implement a marketing program detailing the services and assistance available from the Case Manager (to students, faculty, and staff).
- Ability to work directly with people diverse racial, ethnic and socio-economic backgrounds.
Minimum Qualifications:
MINIMUM QUALIFICATIONS
- Bachelor’s degree required, Master’s preferred.
- Licensed Social Worker or related licensure and/or advanced certifications preferred
- Experience with case management, social work, and community resourcing required.
- Knowledge of county and regional social service providers such as housing, public health, mental health, and justice systems preferred.
- Knowledge of crisis intervention strategies required.
- Ability to work directly with people from diverse racial, ethnic and socio-economic backgrounds.
- Ability to adapt to changing work demands; demonstrate sensitivity and competence addressing multicultural mental health and social work issues.
- Knowledge of mental health and basic needs services for college students preferred.
- Valid driver’s license, insurance and access to private vehicle required; travel to all campuses required.
- Experience conducting presentations to both large and small groups.
- Excellent verbal, listening, writing, problem-solving, and interpersonal communication.
