As a Case Manager working in our Disability Employment Service, you will play a crucial role in delivering high-quality, person-centred services to individuals facing complex and vulnerable situations. You will work collaboratively with clients to develop and implement servicing plans that address psychosocial barriers, fostering their journey toward holistic well-being.

Key Responsibilities:

  • Conduct thorough assessments to identify psychosocial, health, and environmental needs.
  • Build and maintain strong, trusting relationships with clients through empathy and active listening.
  • Collaboratively set realistic and achievable goals with clients, breaking them down into manageable steps.
  • Develop and implement comprehensive servicing plans, incorporating a strengths-based approach.
  • Coordinate services with internal and external stakeholders to ensure a holistic support network.
  • Monitor and evaluate client progress regularly, making adjustments to servicing plans as needed.
  • Provide crisis intervention and support during challenging times.

In addition to receiving a salary ranging between $71 000 – $76 000, superannuation, you’ll also receive 17.5% leave loading and access to the full $15,900 salary packaging benefits. There is also an option for additional purchased leave to increase your annual leave entitlements.

You’ll also bring with you:

  • Tertiary qualifications in social work, psychology, counselling, or a related field
  • Proven experience in case management, preferably working with complex or vulnerable populations.
  • Strong interpersonal and communication skills.
  • Cultural competence and the ability to work with diverse populations.
  • Knowledge of community resources and service providers.
  • Ability to work independently and as part of a collaborative team.
