    As a Case Manager, working full time with Uniting’s Disability Employment Service (DES), you will deliver high-quality, person-centred services to individuals facing complex and vulnerable situations to support their journey to secure new work or return-to-work. This is an office-based role where you will work from different Uniting locations, servicing the greater Melbourne southeast region (Collingwood, Prahran, Dandenong, etc.), to support fluctuating caseloads as needed.

    Key Responsibilities

    • Conduct thorough assessments to identify psychosocial, health, and environmental needs.
    • Co-design individual plans with clients that set realistic and achievable goals.
    • Ensure holistic client support by coordinating internal and external referral services.
    • Monitor and evaluate client progress making adjustments to servicing plans as needed.
    • Provide crisis intervention and support during challenging times.

    About you

    You hold strong communication and engagement skills with an ability to ensure mutual understanding occurs in all conversations and communications. You understand the need for KPIs and have worked in an environment that encourages the achievement of targets.

    You’ll also bring with you:

    • Tertiary qualifications in community services, social work, counselling, mental health, or related field
    • Demonstrated experience and high capability in case management, preferably working with complex or vulnerable populations.
    • Proven experience conducting assessments to understand an individual’s needs
    • Demonstrated success in co-designing plans to support an individual’s general and employment goals.
    • Current valid driver’s license
