About the role
We are seeking a dedicated Case Manager for our Greenlight Program!
You will support people making the transition from chronic homelessness and rough sleeping to placement in longer term housing. Additionally, you will provide brief intervention, case management and service coordination.
Responsibilities include
- Case management support is provided to clients with complex needs
- Engage in assertive outreach practice to provide access to support services
- Utilise evaluation framework tools to capture outcomes of the program
- Participate in team meetings and service planning workshops
- Actively participate in staff meetings, team meetings and services meetings
This is a permanent full-time position, based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
What you will have
- Degree qualification in Social Work, Social Science, Psychology, or related
- Relevant experience in Social Services sector, including case management
- Knowledge of homelessness including causes relating to best practice
- Experience working with internal and external stakeholders to maintain new connections
- A Current Australian Driver’s Licence
- Proof of COVID Vaccination
