Description
Case Management:
- Conduct assessments to identify the needs of individuals and families requiring PSS and MHPSS services.
- Develop individualized care plans in collaboration with clients, addressing their specific needs and goals.
- Monitor and review care plans regularly, adjusting them as necessary based on client progress and changing needs.
- Maintain accurate and up-to-date case files and documentation.
Service Coordination:
- Coordinate with MHPSS Coordinators, Outreach Mobile Workers, PSS Facilitators, Psychotherapists, and Psychiatrist Doctors to ensure integrated service delivery.
- Facilitate referrals to appropriate internal and external services, including health, education, and legal assistance.
- Follow up on referrals to ensure clients receive necessary services.
Client Support:
- Provide emotional support and counseling to clients, helping them cope with trauma, stress, and other mental health challenges.
- Conduct regular follow-up sessions with clients to provide ongoing support and address any emerging issues.
- Educate clients and their families about mental health and psychosocial issues, promoting understanding and reducing stigma.
Community Engagement:
- Conduct outreach activities to identify individuals in need of support and to raise awareness about available services.
- Build and maintain relationships with community leaders and other stakeholders to enhance service delivery and community support.
- Participate in community meetings and events to promote the work of the MHPSS center and gather feedback from the community.
Reporting and Documentation:
- Prepare regular reports on case management activities, client progress, and service delivery outcomes.
- Maintain confidentiality and ensure data protection standards are upheld in all documentation and reporting.
Qualifications & Preferred Skills
- Education:
- Bachelor’s degree in social work, Psychology, Counseling, or a related field. A master’s degree is preferred.
- Minimum of 3 years of experience in case management or social work, preferably in a humanitarian or development context.
- Experience working with individuals affected by conflict, trauma, or displacement is highly desirable.
- Strong interpersonal and communication skills.
- Ability to work effectively in a multidisciplinary team.
- Excellent organizational and time management skills.
- Proficiency in using case management software and tools.
- Fluency in Arabic is required; proficiency in English is an asset.
- Empathy and sensitivity to the needs of vulnerable populations.
- Ability to maintain confidentiality and handle sensitive information.
- Strong ethical standards and commitment to humanitarian principles.
