- Full time, ongoing position;
- $85,119.55 – $91,597.19 p.a. + 11.5% super

ABOUT THE ROLE?
Join our dynamic team as a Case Manager, where you’ll be the cornerstone of our Eastern Metropolitan Region’s Case Management team. As a vital member of our Home Care Packages team, you’ll be at the forefront of assisting and empowering individuals with complex care needs.
Your role won’t stop there. You’ll be the champion of engagement, forging connections with all essential stakeholders. Your mission? To craft personalised, tailor-made care packages, all while consulting with individuals, their devoted caregivers, service providers, and other crucial figures in their lives. Your expertise will ensure that our clients receive the exceptional care they deserve, right in the comfort of their homes.
REQUIREMENTS
- Tertiary qualifications in relevant health / social sciences;
- Demonstrated commitment to the Purpose and Principles of Annecto;
- Previous experience in case management in a community setting;
- Demonstrated ability to work efficiently with culturally diverse groups;
- Demonstrated ability to work autonomously with a high degree of initiative;
- Excellent communication skills, both written and verbal;
- Understanding and commitment to implementing Consumer Directed Care Initiatives and person centred approaches;
- Current unrestricted Victorian Driver’s license.
- NDIS Workers Screening Check;
- Must have been vaccinated against COVID-19 in line with the Victorian Government Public Health Directions.
WHAT WOULD BE GREAT TO HAVE?
- Knowledge of local / regional community resources / professional services to support older and vulnerable Australians;
- Experience working in a similar capacity in a community based not-for-profit organisation.
- A language other than English, in particular Italian and Greek
