About the role
At CatholicCare, we currently have an opportunity available for a skilled & experienced Case Worker to support individuals and families experiencing homelessness and are seeking refuge in crisis. Our Case Workers have high levels of compassion, empathy, patience, and tolerance. A humanistic approach and a fun personality are a must, but you also need to be able to think fast, deliver practical and emotional support and be solution focused.
This is a per permanent full-time position based out of our Dom’s Place in Hornsby with some days requiring to be worked at our Waitara location. Dom’s Place is a drop-in centre, operating as a ‘one-stop-shop,’ an inclusive space where women, men and children can feel safe and connected. Some people pop in to get a meal, wash their clothes and chat with a social worker to address housing needs. Others come and play a game of pool, use the computers and connect with employment pathways. We offer case management, counselling, groups and mental health supports, as well as referrals to other services as needed. Other local services also operate from Dom’s Place including legal, housing, cultural, financial and medical services.
The main objective of the role is to provide case management intervention for clients experiencing homelessness or who are at risk of homelessness. The role will include addressing homelessness concerns through building client capabilities, increasing safety and promoting change through an empowerment client centered model.
It is a very hands-on job, that at times can be challenging but massively rewarding. We want clients to feel safe and supportive, but to also have a positive experience with our service and organisation.
Key Responsibilities:
- Providing case management support that is consistent with trauma informed practices.
- Conducting assessments, developing case plans in consultation with each client, conducting regular case plan reviews specific to the needs of the individual.
- Ensure appropriate referrals are made to address risk factors such as mental health, drugs and alcohol and other contributors.
- Liaising/working closely with internal and external services/agencies involved in the care and support of individual clients to ensure the best outcomes possible.
- Assisting clients in developing basic living skills that will prevent them from returning to a state of homelessness & referring to other appropriate services including but not limited to mental health, AOD services, family support services.
- Assist and advocate for clients in meeting practical needs such as housing, courts & other service providers.
- Maintaining accurate and up-to-date documentation of all client interactions.
- Working across multiple sites including our drop in centre and assisting with the operating of the drop in centre with the team.
- Providing outreach support to clients when required at various locations based on client need.
To be successful in this role, you will have:
- Tertiary qualifications in a Social Work, Psychology, or related areas.
- Minimum 2-3 years’ experience in Casework or Case Management working in homelessness (preferred but not essential).
- Ability to demonstrate networking and advocacy skills with both internal and external stakeholders.
- Ability to provide strong and respectful advocacy in situations of high pressure and competing agendas.
- Ability to manage a crisis or safety situations and make sound decisions under pressure.
- A Working with Children Check and Criminal History Check.
- A NSW Driver’s Licence and own insured vehicle.
