The need-to-know…

The Case Worker – Youth Justice is responsible for the delivery of the Hope Agreement. You will be an integral part in the collaboration with other consortium partners by attending meetings, sharing good practice, collating data, reporting upon outcomes and promoting the service.

In this role you will manage a case load of between 5 – 8 young people, dependent on case complexity. You will help young people to address complex issues that are negatively impacting their life, skills, health and development. This includes, mentoring & supporting young people to access health services such as registering for a GP, dentist or psychologist, facilitating referrals to other services and increasing leisure and recreational opportunities.

A typical day could look like…

  • Attend meetings & collaborate with other consortium partners.
  • Establish & maintain links with relevant community organisations.
  • Provide case management support including face to face in client’s homes or in detention.
  • Provide transport to clients to support adherence with community orders.
  • Monitor & review support plans.
  • Resolve complaints & compliments inline with organisational requirements.
  • Assist with recruitment & development of staff.

This role is part-time, working 19 hours per week across 3 days, working days can be negotiated, however, they must be on business days (no weekends). You will be required to travel to various metro locations across Perth to meet clients, transport clients & liaise with external agencies in addition to attending various Rise offices, with some working from home. A work vehicle will be available for use when you travel to meet clients & external agencies.

The need-to-have…

  • Relevant qualifications in Community Services and/or working towards the qualification and/or relevant experience in a related role.
  • Well-developed communication and interpersonal skills evidenced by your experience of working with young people.
  • Demonstrated knowledge and understanding of the issues impacting on young people at risk.
  • Ability to organise youth events and group activities.
  • Ability to work with minimal supervision in a community setting.
  • The ability to manage and prioritise your own workload.
  • Intermediate computer literacy.

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker Orientation Module / Working with Children Check / Department of Justice Clearance / current driver’s licence.

Costs for new checks will be covered and/or reimbursed by Rise.
