Position Summary:

The Client Wellness Navigator – older adults position supports People Living with HIV (PLWH) and hepatitis C (HCV), ages 55 and over, to facilitate entry or re-entry into needed medical care and services. This individual identifies barriers to care, provides individualized supportive services, and collaborates with clients and case managers to eliminate those barriers. Client Wellness Navigators provide wrap-around supportive services in collaboration with Integrated Case Mangers, Housing Case Managers, Infectious Disease Providers, Mental Health Providers, Harm Reduction Specialists, and other community supports as needed.

Duties and Responsibilities:

  • Provide health navigation services to PWLHIV/HCV with high barriers aged 55 and over.
  • Must have the ability to work from home and in the field.
  • Coordinate patient appointments for HIV and/or HCV medical care and services.
  • Provide health education, close monitoring, and continued motivation to assist patients in navigating the healthcare and social service system.
  • Provide patient education about the correlation between viral suppression, virus transmission, and optimal health outcomes.
  • Provide transportation and attend client appointments on an as-needed or as-desired basis.
  • Meet with clients periodically to track adherence to treatment.
  • Participate in agency outreach events.
  • Utilize practice techniques as trained by Damien staff (such as Motivational Interviewing and strength-based skill-building approaches) to meet clients where they are and encourage them to seek needed care and services.
  • Make recommendations for referrals for psychiatric assessments/services, substance abuse counseling/treatment, mental health services, case management, career development, housing, food pantry, dental, vision, legal, and transportation.
  • Provide clients lost to care with targeted outreach, mailing, and/or phone calls as appropriate to facilitate reentry into care.
  • Complete needs assessment at initial contact and every 90 days thereafter.
  • Active participation in staff meetings, training, professional development opportunities, community fairs, and public relations opportunities.
  • Document all work with clients, as required by funder standards, and in a format agreed to by Damien.
  • Participate in periodic staffing with Damien departments.
  • Other duties may be assigned.
  • Performs special assignments and other work, on an as-needed basis.

Education and/or Experience

High school diploma or graduation equivalency degree (GED) required; bachelor’s degree preferred. Knowledge of HIV/AIDS treatment and care required. Demonstrated experience working with diverse populations required. Must have a valid driver’s license, reliable transportation, and proof of insurance. Lived experience with the focus population preferred. Lived experience a plus. Bilingual in Spanish/ English a plus.

Knowledge, Skills, and Abilities:

Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, achieving deadlines and with superior product results. Strong calendaring, organizational, and document management skills required. Proactive approach to solving problems and communicating to leadership. Knowledge of office management systems, procedures, and office equipment required. Excellent time management skills and ability to prioritize work.

  • This is a hybrid field work/work-from-home position reporting to multiple satellite locations.
