About the role
- Assess, support & monitor the mental health of participants using a trauma-informed, recovery oriented approach – this will include completing comprehensive clinical and non-clinical assessments
- Promote & support the physical health of participants by undertaking health screenings & supporting to follow up with essential health appointments
- Coordinate care & support participants to attend appointments with a wide range of health professionals including GP’s, Hospital and Health Services, Psychiatrists & Psychologists
- Actively seek referral pathways & provide advocacy for participants to access essential services
- Liaise & build strong connections with a wide range of external stakeholders including Psychiatrists, GP’s, and Hospital and Health Services in order to improve access for participants
- Assist with the development & implementation of shared support plans with the wider multi-disciplinary team (Social Workers, Peer Workers, Psychologists & NDIS Readiness Facilitators)
- Participate in case discussions, consultation with team members & clinical review regarding participant’s ongoing support needs
Role criteria
- Must have one of the following: AASW eligibility as a Social Worker OR General/Provisional AHPRA registration as a Psychologist OR General AHPRA registration as an Occupational Therapist or Mental Health Nurse
- Experience in providing coordination of supports for people living with severe & complex mental health concerns
- A comprehensive understanding of the correlation between mental health & physical health & a demonstrated ability to assess & navigate support services for these
- Demonstrated knowledge & experience in supporting participants through the main duties & responsibilities outlined in the position description (must speak to this in the cover letter)
- A well-articulated framework for practice that utilises a trauma-informed & recovery-oriented approach
- Exceptional interpersonal & communication skills
- Demonstrated knowledge & experience in computer skills, particularly data management systems, word processing skills & Microsoft Outlook management
- demonstrated ability to contribute as part of a team to provide a safe, equitable & rewarding workplace
Additional requirements
- Working with Children Card and NDIS Clearance Check (or ability to obtain one)
- National Criminal Record check – disclosable outcomes considered
- Drivers Licence, preferably with own vehicle insure with minimum third party insurance

About the Recovery and Discovery Centre (RADC)
Communify’s Recovery & Discovery Centre is a Mental Health Hub that is designed to provide integrated, multi-disciplinary supports for people who live with severe and complex mental health concerns that are having a significant impact on daily life and wellbeing. The recovery & Discovery Centre is funded through the Brisbane North PHN and delivers support across three key funding streams: Individual and Group Psychosocial Support (including peer support), Clinical care Coordination, and Group Psychosocial Therapies. With a framework based on The Recovery Model, the support that is provided is strengths-based, client-centred and trauma-informed.
