Job Summary
The UCSF SBIRT Collaborative Care Program develops, sequentially implements, and evaluates a multidisciplinary, team-based approach to screening, brief interventions, brief treatments, and referrals to specialty care for alcohol, opioid, and other drug use in both adult and adolescent primary care. The SBIRT-CCP Clinical Social Worker is part of a stepped-care program focused on integrating screening and management of at risk substance use and substance use disorders (SUD) within the primary care setting. The program draws from the University of Washington Collaborative Care Model, in which the Licensed Clinical Social Worker is embedded into primary care and trained to provide validated, symptom based screening of common behavioral health conditions (e.g.: AUDIT, DAST, TAPS1/2), evidence-based care coordination, brief behavioral interventions, and to support, through monitoring and coaching, psychopharmacological treatments initiated by the PCP, psychiatrist, and/or addiction medicine physician. The LCSW is integral in linking patients to behavioral health resources on behalf of primary care and therefore impacts all levels of the business and clinical care operations. The LCSW provides social work assessment, crisis intervention, therapeutic interventions, consultation and education.
As part of a multidisciplinary treatment team, under general supervision, the Clinical Social Worker’s duties include but are not limited to providing transitional and comprehensive mental health services — including case management, group and individual therapy, and crisis intervention — to a caseload of adults with SUD and other common behavioral health conditions (e.g. anxiety) being seen at UCSF Health adult primary care. Treatment modalities may vary but will emphasize brief cognitive-behavioral therapy (CBT) and the community reinforcement approach (CRA) with a strengths-based focus. Interventions may include motivational interviewing and harm reduction. Incumbent will complete psychosocial/functional assessments, mental status examinations, and comprehensive treatment development leveraging a treat-to-target Collaborative Care model. The Clinical Social Worker coordinates treatment among various agencies and advocates for patients to link with needed resources. Attends weekly staff and treatment planning meetings held with Primary Care Services and/or the Office of Population Health.
Demonstrates the ability to work with any patient regardless of race, gender, religious affiliation, cultural believes, lifestyle, disease process or treatment plan within an interdisciplinary team that may include a psychiatrist, attending physician, resident physician, psychologist, clinical nurse specialist and/or nurse practitioner, health navigator and other members of the outpatient practice staff.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% – 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.
For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Department Description
The Department of Medicine (DOM) is the largest department in the School of Medicine (SOM) here at UCSF, with more than 600 full-time faculty. The mission of the Department includes research, teaching, patient care, and public service. The Department has a decentralized organizational structure across four locations (Mission Bay, Parnassus Heights, San Francisco Veterans Affairs Health Care System, and the Zuckerberg San Francisco General) and approximately 38 divisions. UCSF DOM has consistently ranked among the top four departments of medicine in the country. Authority and accountability of the Department rests with the Department Chair (leads Parnassus Heights site) and also directs the Vice Chairs/Chiefs at the SFVAHCS and ZSFG.
The Division of General Internal Medicine (DGIM) is one of the divisions within DOM and SOM. DGIM, one of the larger Divisions in DOM, is home to a spectacular group of faculty, learners, and staff committed to outstanding patient care, research, and education. DGIM is a national leader in clinical care, education, and research, and our mission is to provide high quality and accessible clinical care, train the future leaders of primary care, and to conduct cutting-edge research in wide-variety of disciplines, including the aspects of health outcomes related to socioeconomic and ethnic factors. Required Qualifications
• Master’s degree from an accredited school of social work, possession of a clinical social worker license issued by the California Board of Behavioral Science Examiners, and three years of post-master’s experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position;
• This position requires flexibility to orient and work at all UCSF Medical Center locations.
• Three years of experience in a healthcare setting required and hospital setting preferred.
• Familiarity with an electronic medical record program and/or Microsoft Office, Excel, Word and Outlook
• Knowledge and abilities essential to the successful performance of the duties assigned to the position.
• Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
• Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high volume work environment.
• Ability to work effectively and collaboratively with interdisciplinary teams.
• Demonstrates case management skills and experience in the community health care delivery system.
• Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems and teams of health professionals.
• UCSF Medical Center requires all Advanced Health Professionals (AHP) to be credentialed through its Office of Medical Affairs & Governances (OMAG) in order to practice and function in their clinical role. This applies to both adult and pediatric AHPs in the inpatient and outpatient clinical settings at all Langley Porter and UCSF Medical Center sites. Credentialing and approval of the Standardized Procedures takes between 60-90 days and must be completed as soon as possible.

• Second language skills: Spanish or Cantonese
• Knowledge of the collaborative care model for SUD management within the primary care setting; experience providing therapy or case management services within the primary care clinic setting
License/Certification
