The Opportunity

IPC Health’s Care Finder service supports older vulnerable communities who are eligible for aged care services and experience barriers and challenges accessing services and supports, including having no family or representative who is willing or able to support them. The program provides specialist and intensive assistance, through assertive outreach and engagement strategies, to support and assist people within our target populations understand and access aged care services and connect with other relevant supports in the community, including My Aged Care.

The Program operates across Melbourne’s West including Brimbank, Maribyrnong and Hobsons Bay and Outer West including Wyndham, Melton and Moorabool Local Government Areas.

This is a Full Time Fixed Term position until 30 June 2025 (Funding Based Activity Project) and is classified at a Social and Community Services Employee Level 3 offering $73,795 – $79,135 per annum under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022.

You will make a difference by

  • Developing and implementing engagement strategies that enhance understanding and connection with the Care Finder service, ensuring approaches are dynamic and responsive to community needs.
  • Build and maintain relationships with diverse communities, including older persons eligible for aged care services and support, including priority groups being CALD community, people with a disability, Aboriginal and Torres Strait Islander community and those experiencing disadvantage.
  • Promoting the Care Finder Service through assertive outreach activities, events and community forums aimed at engaging identified client groups, key stakeholders and potential intermediaries.
  • Collecting, and recording data, and providing regular reports that reflects community engagement outcomes aligned to program requirements.

To succeed you will need:

  • Qualifications in community development, community engagement, community services, health promotion or related area; or extensive relevant experience and specialist expertise
  • Understanding of and a minimum of two years demonstrated experience in targeted community and stakeholder engagement, with particular focus on older vulnerable communities including , CALD communities, people with a disability, Aboriginal and Torres Strait Islander community and those experiencing disadvantage.
  • Demonstrated experience in strategic planning to achieve program outcomes.
  • Demonstrated experience in data collection, recording and reporting to reflect outcomes
  • Excellent technical skills in Microsoft Office suite and client management systems, experience with TrakCare is highly desirable.
  • A current full Victorian drivers licence and Working with Children’s Check
