About the role
The Sub-Acute short stay Adult Step-Up Step-Down (ASUSD) residential service, delivered in partnership with QLD hospital networks and the local health service, supports adults with a psychosocial disability up to 28 days transitioning from a hospital setting back into the community or to prevent admissions whilst residing in the community. Clients are supported 24/7 to achieve their recovery goals, strengthen relationships with families and carers, develop community networks, build confidence and capacity to live safely in the community. ASUSD works within a recovery focused, clinical treatment framework providing personalised one-on-one support, shared supports and group activities based on client’s strengths and goals as identified in their recovery plan.
We are seeking a resilient, compassionate Community Mental Health Practitioner – Aboriginal & Torres Strait Islander for a 12-month fixed term, part time (30.4 hours per week) role on Gimuy Walubara Yidinji Country. Your will deliver person-centred support and recovery-oriented services to support clients facing mental health challenges achieve their recovery goals, develop life skills, and strengthen connections with the local community and elders.
Key responsibilities
- Deliver high quality services and facilitate positive educational and social experiences.
- Support clients to return to a community of their choosing, live independently and enjoy a meaningful life through a staged approach to recovery.
- Provide information and mentoring to clients, including Aboriginal and Torres Strait Islander clients to reconnect with local communities and support networks, establish relationships with families and carers, and access to relevant services.
- Build trusted relationships through strengths focused engagement with local communities, kinship groups and elders and provide stakeholders with awareness of health outcomes for Aboriginal and Torres Strait Islander peoples.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field, or the ability to obtain the minimum qualifications.
- Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
- Expertise in working with Aboriginal and Torres Strait Islander peoples with mental health issues, complex needs and interacting with families and carers.
- Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
