About the service

The Adult and Older Adult Mental Health and Wellbeing services, also refer to as “Locals”, are a key recommendation from Victoria’s Royal Commission into the mental health service system’s transformation. The Locals will act as the ‘front door’ service to the mental health system and will provide a continuum of care and support, including clinical interventions, wellbeing supports, and therapeutic support through an integrated mental health and alcohol and other drug framework approach. These services will have a ‘no wrong door’ policy, ensuring accessibility to all individuals seeking support. 

About the role

We are seeking a self-motivated, empathic Community Mental Health Practitioner. This is a permanent full time or part time (22.8 hours per week) with a 7 day roster opportunity to work within the Dandenong area. You will deliver one-to-one individualised support, intake and assessment, outreach support, service navigation and care coordination. In line with My Better Life model and organisational values for people with mental health and/or alcohol and other drugs challenges.

You will play an integral role in working collaboratively with consumers to achieve their wellbeing and recovery goals through, one-to-one support, and group work facilitation to enhance recovery, personal growth and activities of daily living. 

Key responsibilities

  • Provide information and mentoring to clients to establish or re-establish relationships with families, carers and supporters, create community networks and access to relevant services.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the needs and preferences of people being supported.
  • Deliver high quality in person and outreach services and facilitate positive educational and social experiences.
  • Support people to remain in the community and promote their wellbeing.
  • Plan and deliver group programs that support people to build their skills and focus on their wellbeing.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to people‘s stated needs and preferences.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, Community Services or other professional field as designated by Mind.
  • Experience and expertise in working directly with people with mental health issues, AOD challenges, complex needs and with their families and carers.
  • Experience of providing empathic person-centred active supports.
  • Demonstrated knowledge and experience in the application of theoretical approaches, practices and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioural therapy, motivational interviewing and harm reduction therapy.
  • Relevant experience in facilitating group activities alongside people experiencing mental health and/or AOD challenges.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
