About your new role

The Commonwealth Psychosocial Support (CPS) Service offers individual coaching and group activities to support recovery for people with severe mental health within their local communities, who are not eligible for the NDIS.

CPS supports consumers by:

  • Providing flexible recovery orientated 1:1 support
  • Designing and facilitating group programs
  • Providing support to consumers wanting to apply or reapply for NDIS
  • Actively engaging carers and care teams where appropriate

The CPS Service is funded by the Northern Melbourne Primary Health Network (PHN) and covers the following regions; Darebin, Hume, Merri-bek, Yarra, Melbourne, Moonee Valley and Macedon Ranges. The CPS Service is supported by internal programs including NDIS Interface who solely supports consumers with NDIS applications, and Integrated Chronic Care (ICC) who support consumers with co-existing chronic physical health conditions.

As an organisation who understands the importance tailoring support provision to the communities it works within, Neami is seeking a CRSW or PSW who has established knowledge and connection in the Macedon Ranges area. As the Macedon CRSW/PSW in the CPS team you will provide short to medium term community based psychosocial support to a caseload within the region.
