Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Services division. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.

About the Role

The Connected Community Pathways (CCP) Project Officer will work collaboratively with the CCP Coordinator and Kalwun Health Services staff to deliver the 3 CCP initiatives of – Cancer Screening; Vulnerable Youth; and Palliative Care and End of Life Services. This is a full-time role until 30 June with the possibility of extension.

Position Duties

  • Working collaboratively to deliver the 3 CCP initiatives in collaboration with the CCP Coordinator and Kalwun Health Services Staff.
  • Collaborating with local Indigenous health services and mainstream health services in a partnership approach for the delivery of primary care services
  • Improve the capacity of mainstream primary care services to deliver culturally appropriate services to Aboriginal and Torres Strait Islander people
  • Assist in facilitating community engagement through yarning circles, women’s/men’s groups, community days, promotional events.
  • Cancer screening
    • supporting access to, and update of women’s cancer screening
    • Assisting the CCP Coordinator in delivering targeted strategies for women who are under-screened or have never been screened for women’s cancers
  • Vulnerable Youth
    • Provide moderately intensive case management for Aboriginal and Torres Strait Island young people aged 10-17 years with complex transition care needs
    • Assist the CCP Coordinator in the roll out of pathways to support young people as they navigate between hospital and community-based healthcare services, education, child protection, justice, QPS and other providers and sectors.
  • ​​​​​​​Palliative Care and End of Life Services
    • ​​​​​​​Walk alongside individuals and families from the time of a life-limiting diagnosis to a time following death when the family no longer requires more intensive supports
    • Provide education, community resources, and non-therapeutic support to Kalwun clients

Essential Selection Criteria

  • Minimum Certificate III in the Health or Human Services sector or similar
  • Demonstrated understanding of issues affecting Aboriginal and Torres Strait Islander people and their health
  • Providing empathetic and compassionate care to members of the community
  • Proven ability to provide cultural, non-therapeutic support and guidance when required – through community, the hospital system and other applicable services
  • Ability to advocate for patients negotiating the health care system, child protection, justice, QPS and other providers and sectors
  • Experience in Aboriginal and Torres Strait Islander Primary Care and/or Youth Services is highly desirable.

Additional Information

  • This position requires the successful applicant to satisfy a national criminal history check and hold a current drivers’ license.
  • A Blue Card for working with children is a mandatory requirement. The ‘No Card No Start’ law applies to this position – a Blue Card must be obtained prior to commencement.

It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 that applicants are Australian Aboriginal or Torres Strait Islanders.
