Primary purpose of the role

As a Disability Support Worker, you will play a crucial role in delivering services to young people we support across 3 accommodation facilities in the Sutherland Shire to enable them to participate in their community, achieve their goals and live their best life. You will lead your team to deliver high standards of practice while embedding and living your passion for social inclusion for people living with disability.

Today, we are a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.

Key Responsibilities

  • Undertake a range of clearly defined support duties in accordance with individualised health care plans to assist participants to remain safe, healthy and make decisions for themselves within their assessed capacity or in conjunction with guardians who have decision-making authority.
  • Promote and role-model delivery of support in accordance with individualised health care plans to assist clients to remain safe, healthy and make decisions for themselves within their assessed capacity or in conjunction with guardians who have decision making authority.
  • Identify and report issues that affect the implementation of Individual Support Plans to inform any need to amend the plan to meet participants individual needs.
  • Complete mandatory documentation accurately and within given timeframes to meet operational requirements.
  • Provide and promote timely reporting on incidents, near-misses, risks and complaints in accordance with Achieve Australia policies and procedures and legislative requirements and participate in any investigations as required.
  • Use positive behaviour support and active support approaches when working with clients and comply with the use of restrictive practices.
  • Contribute to building and maintaining strong internal and external stakeholder relationships, (including families and carers), to address practical issues, enhance operational efficiency and meet client’s requirements.
  • Contribute to a culture of continuous improvement, active support and client safeguarding by demonstrating and encouraging practices to protect clients, children and other vulnerable people from harm and neglect and help maximise positive client outcomes.
  • Participate in staff briefings and undertake training to ensure compliance with the organisation’s policies, processes, and procedures, to meet the NDIS Practice Standards, Quality Indicators, and all regulatory and legislative requirements.
  • Maintain confidentiality and professional conduct at all times to ensure professional boundaries are respected and adhere to behaviour and appearance standards in compliance with Achieve Australia’s Code of Conduct and Dress Code.

About you

  • You are passionate about making a positive impact in the lives of young people with disability.
  • You have understanding of trauma-informed practice and behaviour support and management.
  • You have exceptional interpersonal and communication skills with the ability to build rapport and establish positive relationships.
  • You have empathy, patience and genuine passion for supporting individuals with disabilities.
  • You have highly organised with the ability to prioritise and manage multiple tasks effectively.
  • You have relevant tertiary qualifications or equivalent in Disability, Health, Nursing or other related field.
  • You have demonstrated understanding of NDIS, the principles of Active support and human rights-based approach to service delivery.
