Purpose of the role

Achieve has been providing services to people with disability since the 1950s. Today, we are a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.

About the role

In this role, some of the key responsibilities include:

  • Undertake a range of clearly defined support duties in accordance with individualised support plans (e.g. health care plan) to assist participants to remain safe, healthy and make decisions for themselves within their assessed capacity or in conjunction with guardians who have decision-making authority.
  • Identify and report issues that affect the implementation of IP to inform any need to amend the plan to meet participants individual needs.
  • Complete mandatory documentation to meet operational requirements.
  • Comply with the organisations policies, processes and procedures to meet all regulatory and legislative requirements.
  • Maintain confidentiality and professional conduct at all times to ensure professional boundaries are respected and maintained.
  • Adhere to behaviour and appearance standards in compliance with Achieve Australia’s Code of Conduct and Dress Code.

About you

  • Relevant qualifications (minimum Cert III in Disability or Individual Support) OR
  • Experience in another specialty you can bring to the program, and a willingness to complete Cert III
  • Sound interpersonal, role modelling and customer service skills
  • Ability to problem solve and adapt to changing environments
  • Understanding and commitment to the Disability Service Standards and WHS
  • Understanding of human rights and the challenges facing those with a disability
  • Current NSW Driver’s Licence and
  • Experience in working with and supporting adults deemed as having significant complexity in their lives
