We are seeking an experienced Social Worker to join our AWLD Transition Service team.Benefits of the role:

Responsibilities:

• You will play a crucial part in assessing and supporting vulnerable individuals during their transition to adulthood
• You will focus on supporting young adults aged 18-25 with social needs, including those with learning disabilities, autism, and physical disabilities
• Legally literate and capable of completing Community DoLS assessments
• Experience working with young people with learning disabilities, autism, and physical disabilities.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
