Are you passionate about falls prevention and looking for a new challenge? We are looking for an innovative and motivated clinician (Nurse or AHP) who is passionate about preventative care to join the Professional Practice Team, working across the Community Care Group.Following the success of a two-year preventative care project which measured a 78% reduction in avoidable falls related admissions across 11 care homes, this role is now advertised on a substantive basis to support the prevention of falls in the community.We would love to hear from you if you feel this is a post you would be interested in.Main duties of the jobFor more detail about the main duties of this role, please see the job description section below. This will enable the successful applicant to support service developments, reduce demand on NHS services and develop preventative options and pathways for service users. This will involve the development of close working relationships with internal and external community services and the voluntary sector and support education initiatives.
