Communify is in partnership with Act for kids preventing and treating child abuse and neglect. The Intensive Family Support service offers an extension to our multidisciplinary teams, providing services to the marginalised and vulnerable families in our community. Partnering with Act for kids, means we can work with families at risk by providing information, advice and referral services and intensive family support with practical skill development.

We need a skilled and qualified professional to fill our full time position to enable us to do our important work.
We are currently looking for a Family Support Practitioner to work in the inner northern suburbs of Brisbane in our Intensive Family Support service on an ongoing permanent contract.
The role of the Family Support Practitioner is to deliver outreach family case-management to families, caregivers, kids and young people.
We need someone who:
- Engages families, obtaining consent and developing case plans in conjunction with families addressing unmet needs and child protection concerns.
- Has a high level of cultural competency and ability to work well with families from all different cultures.
- Works collaboratively with other professionals, services and agencies to ensure families goals are achieved.
- Relevant tertiary qualification in human services and,
- Demonstrated experience with delivering intensive family support.
